Dr Ward is the owner of ABC Vision Source and graduated from Pacific University College of Optometry in 1993 and completed residency with the Veterans Administration Hospital in 1994. She has been in practice since 1994 and in 1997 became an adjunct clinical faculty member of Pacific University College of Optometry.

Dr Ward was the Oregon Chapter treasurer of the Volunteer Optometric Services to Humanity from 1995 to 2005 and has served on two committees as a member of the Oregon Optometric Physicians Association. Dr Ward is also an associate member of the Optometric Physicians of Washington, and a member of the Portland Metro Optometric Society and received service recognition in 1999. Dr Ward specializes in primary care and ocular disease and has a long history of pre and post laser vision and cataract surgery care.

Wesley CrocketDr. Crockett grew up in Southern Arizona and did his undergraduate studies at Northern Arizona University where he received a degree in Spanish with minors in Biology and Chemistry. Dr. Crockett moved to Oregon in the summer of 2007 to attend Optometry School at Pacific University. Upon completion of Optometry school Dr. Crockett joined the staff of ABC Vision Source in the summer of 2011.

Dr. Crockett is fluent in Spanish and loves working with the spanish speaking community. He is married and has three children. He enjoys fishing, hunting, camping and all modes of outdoor recreation. He is excited to be in Oregon and be part of ABC Vision Source.

The contract for Primary Care Services is an annual agreement with the Provider.
Members are responsible for all fees for one full year.

Primary Care Services Include:

  • Annual eye exam and 1 injury/illness exam
  • Illness/injury exam to include: pink eye, flashes, floaters
  • Disease prevention and optimizing health consultations

Additional Reduced Rate Fees:

  • Comprehensive exam includes: dilation, medical issues such as diabetes care, loss of vision etc. Call for more details on your condition
  • Contact, prescription and fitting - $50-$135, Includes: frames, lenses, treatment, 1 yr warranty
  • Any special eye diseases such as cataracts, glaucoma, or complications from diabetes will require special attention to eye care. We can effectively manage conditions and prescribe the appropriate medications or arrange for eye surgery. Co-manage with surgeons and hospitals
  • Any problems that are noted may need referral to another care provider. Other lab tests if requested by the provider will incur additional costs
  • Any charges not covered under the co-op plan will be payable at the time of the visit
